Transitional Care Management
Our Transitional Care program was designed to keep you out of the hospital. Our 30 Day Program starts when you leave the hospital or rehab and covered 100%. No matter your insurance. Decrease your recovery time and increase your access to the best providers in medicine.
Patients are followed by providers who hold decades of experience in Hospitals, Rehab Centers, and Medical Clinics. After 30 days in our care, we guarantee you will not only feel better, but will have the tools and resources needed to stay healthy.
We provide the following:
- Review discharge materials with you and educate on next steps.
- Confirm prescriptions are filled and correctly administered.
- Help Scheduling follow up appointments.
- Coordinate specialist referrals and appointments.
- Create health goals and help you achieve them.
- Connect you with community services and resources.
- Prevent avoidable readmissions and relapses.
- Help you build the confidence to manage your healthcare at home.
